This form is not random. It is designed to get the most useful information in the easiest way. This is not like a govt. form for files, this is a working document.
Using simplicity, some political correctness may be sacrificed. Knowing the gender of the sender simply helps with addressing you. City, State, and country are for our marketing records. All the other items are fairly obvious.
